            Individual Nutritional Counseling

     Fee:       Initial 1 hour visit                $92.40

                   Follow-up visits 1/2 hour     $46.20 

Nutritional assessment and counseling provided by registered dietitians for general nutrition and a variety of medical concerns requiring nutritional intervention.

*A referral with diagnosis, indication of medical necessity, and several other requirements must be met. Individual nutrition counseling rates apply. The patient is responsible for and will be billed for a co-pay of 20% of the pre-approved Medicare price for this benefit.
