PLANS OFFERED (30 days to enroll): JeffPLUS Aetna PPO & JeffPLUS Keystone Choice PPO Each features excellent network providers and facilities, but also allows you to seek care outside the network if you wish.
If you decide to go outside the network for care and services, our out-of-pocket costs are higher. You do not need to select
a primary care physician and you can see a specialist without a referral. There are copays for physician visits and you do
not have a deductible for certain services.
JeffPLUS Aetna HMO & JeffPLUS Personal Choice HMO Each features affordable access to networks that include the region's most respected health care provides and facilities.
The JeffPLUS Aetna HMO gives you access to JeffPLUS doctors and facilities in the Aetna network of providers. The JeffPLUS
Keystone, which is administered by Independence Blue Cross, gives you access to JEffPLUS providers and facilities in the Keystone
network.
JeffPLUS Aetna CDHP (Consumer Driven Health Plan) A new concept that gives you more control in how your health care dollars are spent and puts more decision-making responsibility
in your hands.

STD Buy-Up Supplemental Insurance

Optional: Employee earning $58,000 or more can increase their monthly dollar amount maximum through the STD Buy-Up Plan. This
buy-up plan coverage gives eligible employees the flexibility of selecting the amount of buy-up coverage (up to 60% of your
annual base earnings on top of the Basic STD Coverage). Coverage is for up to 26 weeks. Must enroll within 30 days of hire
date to be eligible.

| Specified Critical Illness Insurance |
FHCS offers an opportunity to supplement your Medical and Disability coverage by purchasing Insurance Specified Critical Illness Insurance through UnumProvident at affordable group rates. This plan pays a lump sum
benefit directly to you (in addition to any benefits you might receive from other plans) to help you pay direct and indirect
costs associated with a critical illness.

| Profession Liability Insurance |
Temple provides, at no cost, each resident with Professional Liability Insurance (malpractice) during his or her residency.
Coverage is limited to the resident's professional responsibilities performed only at Temple and its approved affiliates and
only while acting within the scope of the graduate training program. Eligible as of start date.

| Countrywide Legal Services |
This plan covers your, your spouse and you dependents children up to age of 23, and provides the following services at no
additional cost. Enrollment form will need to be filled out for payroll deduction and membership to the service.
