□ Official Board Score (COMLEX) Reports from the NBOME

□ Three letters of recommendation from clinical faculty with whom the
applicant has worked over the past 12-18 months

□ Letter of recommendation for current program director (or employer if
no longer in training)

□ List of all dermatology rotations
